Canada Pension (CPP) & Old Age Security (OAS)
Our funeral home provides to families the applications for the CPP Death Benefit and Survivors Benefit. Although CPP uses a formula to determine what the Death Benefit value will be, generally it is 6 X's the recipients monthly payment to a maximum of $2,500.00. The Survivor's Benefit is also calculated by a formula. This benefit is available to the deceased spouse and children that are still in school. We would be happy to assist you by providing notorized documents as needed by CPP.
It is important to note that there is no Death Benefit from OAS and the checks that come at the end of the month are payment for that month and the estate can keep these payments.
On behalf of the family, we send to CPP & OAS, a Notice Of Income Security Programs Client's Death. This form will inform CPP & OAS of the death and will facilitate the cancellation of the monthly payments so that the estate will not have to worry about repaying this overpayment.
Death Certificates
The funeral home will file with the Province of Ontario, the Statement of Death (Form 15) and the Medical Certificate of Death (Form 16). It is possible to apply to the Province for certified copies of these forms but generally they are not required for estate settlement. We will provide to you 10 copies of the Funeral Director's Statements of Death. These are accepted by CPP, insurance companies, banks, lawyers and pension plans. We are happy to provide any additional copies that are required at no cost.
Thank You Cards
If you select a stationary package at the funeral home, it will include thank you cards which can be sent to those who sent you flowers, made charitable donations or performed any other act of kindness.
